Ledgewood Rehabilitation is a short term rehabilitation unit within ARK at Branford Hills. Ledgewood is a separate and dedicated unit designed for those individuals who no longer require acute hospital care but are unable to return home without additional care and services.
Ledgewood provides the necessary rehab and medical programs to help patients reach their maximum potential and return home to their families and communities. We also have an extensive outpatient physical and occupational therapy department to complete the continuum of care for our patients. In order to provide the highest quality of care, our focus has always been on the individual patient. Our compassionate and professional staff uses an interdisciplinary team approach to assess, develop, implement, and review each patient in an effort to reach their maximum potential.
Our clinical programs are designed for those patients with medical complications that inhibit their recuperation. Our nursing staff is trained and experienced to treat patients who have complex medical conditions or require special interventions.
We also address the needs of those residents who require rehabilitation services and programs resulting from Post Surgical Conditions, Cardiac Recovery, Orthopedic Recovery, and Stroke Neurological Rehabilitation.
Our in-house staff of therapists design individual therapy programs that focus on each patient’s functional ability and rehab needs. We emphasize and promote the outcomes you desire by teaching strengthening, independence and activities of daily living.

Physical Therapy
At ARK Healthcare at Branford Hills, we offer a full range of therapy services dedicated to helping you regain the strength and function you need to resume and even improve your daily routines and leisure pursuits. We do this through the efforts of skilled and compassionate professionals with long experience in their respective fields.
Physical therapy focuses on regaining function that has been lost due to injury and/or impairment of bones, muscles, joints or nerve damage. Common problems we treat include pain, balance, walking difficulties, strength, endurance issues, and postural control. Our specialized therapeutic modalities program uses ultrasound, electrical stimulation and diathermy to relieve pain, improve circulation, reduce inflammation, and decrease muscle spasms.
Occupational Therapy
Our occupational therapy program is for individuals of all ages whose lives have been impacted by illness/injury and who need support to participate in everyday life activities. These conditions require specialized assistance in learning skills to enable the individual to lead an independent, productive and satisfying life. Upon referral from a physician, patients will receive an evaluation and a personalized treatment plan designed to meet his or her rehabilitative needs.
We work to restore and improve fine motor skills, visual perception and cognition, and provide training on specialized adaptive equipment to enable self care, employment and independent living.

Chronic Obstructive Pulmonary Disease (COPD) Program:
At ARK Healthcare at Branford Hills we have a specialized COPD Program which offer an interdisciplinary approach for the education, therapeutics and monitoring of our residents with Chronic Obstructive Pulmonary Disease (COPD).
If you have a COPD diagnosis or a related diagnosis such as Bronchitis or Emphysema you would most benefit from this comprehensive program.
Pneumonia Program:
We also offer a comprehensive Pneumonia program for those residents who suffer from pulmonary disease.

The pharmacy is conveniently located on the first floor of our facility. In addition to on-site medication dispensing, ARK Healthcare at Branford Hills offers:

- Comprehensive medication management and education
- Extensive knowledge and experience with Medicare part D and other pharmacy insurance plans
